About the Position:

We are seeking a licensed Occupational Therapist who shares our passion for improving patients' quality of life in the comfort of their own homes. As an Occupational Therapist with Optimal Home Care, you will address the performance of everyday activities and participation in roles and situations that occur in the home and community.

Responsibilities:

  • Provide occupational therapy services to patients in their homes, promoting health and wellness through activities and interventions.
  • Assess patients' needs and develop personalized treatment plans to address their goals and objectives.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes, maintaining accurate and comprehensive records.

Requirements:

  • Active Occupational Therapist license in the state of Colorado.
  • At least one year of experience as an OT in home health, preferably with a similar patient population.
  • Adequate and effective written and verbal communication skills.
  • Adequate and effective interpersonal skills.

Compensation:

$80,000 - $100,000 per year, paid per visit.
