Nurse Care Coordinator

Job Summary:
Henry Ford Health is seeking a compassionate and skilled Registered Nurse to provide high-quality patient care in a dynamic and supportive environment. As a Registered Nurse, you will be responsible for coordinating and delivering patient care, working collaboratively with the healthcare team to ensure the best possible outcomes.
Key Responsibilities:
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Delegate and supervise care and teaching of patients and family members.
- Effectively communicate patient information to the healthcare team.
- Participate in finding solutions for variances from patient goals and ensure implementation of solutions.
Requirements:
- Graduate from an accredited school of nursing.
- Current licensure by the Michigan State Board of Nursing.
- Enrollment in a Bachelor of Science in Nursing (BSN) program within 3 years of hire.
- Completion of a Bachelor of Science in Nursing (BSN) within 5 years of hire.
