As a Hospice/Home Health CNA, your key responsibilities will include:
- Delivering services as outlined in each patient's hospice care plan.
- Assisting with daily living activities such as bathing, grooming, light housekeeping, meal preparation, shopping, transportation, and providing companionship.
- Supporting mobility, transferring, and range of motion exercises.
- Offering medication reminders, documenting patient conditions, and reporting any concerns to a supervisor.
- Ensuring a safe living environment with clear pathways.
- Engaging clients in enjoyable activities such as puzzles, games, reading, and hobbies.
Essential qualifications for the Hospice/Home Health CNA role include:
- High school diploma (or equivalent) and active CNA Certification in the relevant state.
- A minimum of one year of experience as a CNA.
- Experience in hospice care.
- CPR certification.
- Valid driver's license, auto insurance, and reliable transportation.
- A compassionate nature, strong communication skills, and the ability to lift up to 50 lbs.
