About the Opportunity

SouthLight Healthcare is a leading provider of substance use treatment and mental health services in the Triangle area. We are seeking a highly motivated and experienced Community Needs Outreach Coordinator to join our team.


This role will involve working closely with our clinical and medical teams to identify and address the social needs of our clients. The successful candidate will have a strong background in human services, excellent communication skills, and the ability to work effectively in a fast-paced environment.


The estimated salary for this role is $55,000 per year, based on industry standards and market research. Benefits include medical insurance, paid holidays, and PTO hours. We also offer opportunities for professional growth and development, as well as a supportive work environment.


Responsibilities
  • Conduct outreach and engagement activities to identify and assess social needs of our clients
  • Develop and implement strategies to address social determinants of health for our clients
  • Build and maintain relationships with community organizations to provide referrals and support services
  • Collaborate with clinical and medical teams to ensure comprehensive care for our clients
