Job Title: Case Management Coordinator

PHMC is a leader in public health, and we're seeking a skilled Case Management Coordinator to join our Survivor Services department at Joseph J. Peters Institute (JJPI). As a key member of our team, you'll play a vital role in providing high-quality case management services to participants, ensuring compliance with management and stakeholder targets, and overseeing day-to-day support activities for therapists and supervisors.

Key Responsibilities:
  • Develop and implement innovative techniques to serve participants more effectively.
  • Ensure compliance with management and stakeholder targets.
  • Oversee a team of case managers and interns (approximately 3-4).
  • Monitor and manage databases and tracking systems for evaluations, assigned cases, and discharged participants.
  • Lead ongoing quality management of survivor services, including chart audits, treatment plan tracking, and compliance with progress note completion parameters.
  • Train new hires on CRS usage, case management documentation requirements, and timeframes for completion.
  • Complete and provide monthly/quarterly statistical reports and/or reports as requested by management.
Requirements:
  • Bachelor's degree in a field related to social services and/or clinical work.
  • 2-3 years of experience in the field, with experience with human services systems and knowledge of resources available to staff and participants.
  • Preferred: Supervisory experience and working knowledge of SPSS.
