Job Summary

Prestera Center for Mental Health is seeking a dedicated Direct Service Provider to join our team. As a Direct Service Provider, you will engage with consumers daily to provide oversight and direct care services, overseeing their activities in various settings, including community, home, residential, or other treatment settings.

Key Responsibilities

  • Provide assistance and/or monitoring/training to consumers in self-care, self-direction, independence, mobility, and learning, as directed by their service/treatment plan.
  • Perform light household cleaning, plan shopping lists based on menus, and assist in meal preparation or grocery shopping when assigned.
  • Organize and participate in leisure activities with consumers, providing instruction and guidance on appropriate behavior and interactions for the setting involved.
  • Accompany consumers in the community for activities, including medical appointments, grocery shopping, attendance at treatment plan meetings, or other planned outings.
  • Monitor consumers in self-administration of medications or administer medications to consumers if indicated/AMAP certified.
  • Provide education or supportive services to consumers.
  • Obtain and document vital signs as assigned.
  • Document consumer observations and behaviors according to policy.
  • Monitor consumer behavior and address inappropriate behaviors according to the consumer's treatment plan or based on consultation with program leadership.
  • Oversee the safety of consumers and direct emergencies that may arise during shift.
  • Possess effective communication skills and utilize verbal de-escalation skills effectively when needed.
  • Assist in behavioral management or crisis intervention as indicated based on training and consumer service/treatment plan.
  • Transport consumers as needed, both in a Prestera Center vehicle or personal vehicle.
  • All other duties as assigned.

Requirements

  • High School Diploma/GED required.
  • Valid driver's license required.
  • Must attain/maintain certification in Non-Violent Crisis Intervention and CPR/FA during employment.
  • AMAP certification may be required.
  • Experience or prior training working with consumers with mental health disorders is preferred.
