We are seeking a compassionate and skilled Caregiver to provide personalized care services to our participants receiving home care and/or day health center services.
Key Responsibilities- Provide personal care services to participants, including bathing, dressing, and grooming.
- Assist with daily living activities, such as meal preparation and medication management.
- Oversee and manage onboarding and ongoing training for other caregivers.
- Participate in team discussions and care plans to ensure high-quality care.
- Take on more complex cases, working directly with RN/PT/OT, social worker, MD, etc.
- Current Basic First Aid and CPR certification.
- Ability to communicate fluently in English (verbal and written).
- Basic computer skills, including email communication and electronic health record documentation.
- Visual acuity and ability to lift/move weight within individual physical ability.
- Medical, Dental, and Vision coverage.
