As a Registered Nurse Case Manager, you will play a critical role in our team, working closely with patients, families, and healthcare providers to ensure seamless care coordination and exceptional patient outcomes.

Key Responsibilities:

  • Plan, organize, and direct home health care services, from initial evaluation and assessment to care plan development and implementation.
  • Ensure high-quality patient experience and positive health outcomes through a comprehensive and personalized plan of care.
  • Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team, and collaborate with the patient's primary care physician to ensure continuity of care.
  • Provide coaching and education to patients and their caregivers based on clinical diagnosis, empowering them to self-manage their condition, prevent complications, and improve quality of life.

Requirements:

  • Current CA Registered Nurse (RN) license
  • Current CPR card for Healthcare Providers
  • Registered Nurse (RN) experience in Home Healthcare setting within the last 3 years
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
  • Self-directed and able to work with minimal supervision.
