About the Role:The Social Worker Outpatient- MSW will assist patients and their families by obtaining services or initiating referrals to eliminate personal or environmental barriers that may interfere with maximum benefits of healthcare.
Key Responsibilities:- Perform psychosocial assessments to identify individualized needs and develop a plan to address each need.
- Make referrals to appropriate resources within JPS Health Network and within the community.
- Collaborate with Providers, Nurse Case Managers, and Patient Centered Medical Home (PCMH) staff to identify the needs of complex high-risk patients.
- Provide brief supportive counseling to patients and families exhibiting complex family dynamics directly impacting patient care.
- Assist patients and families to cope with and manage life-changing diseases, death and dying, advanced directive education, disabilities, etc.
- Support patients and providers with reviewing and completing various eligibility forms for social programs, funding sources, durable medical equipment, etc.
- Follow-up with patients and families to ensure that services provided are helpful, appropriate, and adequate.
- Aid with care coordination for patients with complex psychosocial needs, including providing services in a variety of settings.
- Master's Degree in Social Work from a program accredited by the Council on Social Work Education.
- 2+ years social work experience, including internship work, with one year in a healthcare setting.
- Social Work licensure through the Texas State Board of Social Work Examiners.
- BCLS may be obtained subsequent to hire.
- Case Management Certification (ACM or CCM).
- Experience working in the post-acute setting (hospice, home health, nursing home, or inpatient rehabilitation hospital).
