Position Overview:

We are in search of a dedicated and compassionate Licensed Practical Nurse to join our team in the role of Care Manager. In this position, you will play a crucial role in reviewing and maintaining the accuracy of patient records.

Your responsibilities will include ensuring that all documentation accurately reflects the patient's health status and treatment needs.


Key Responsibilities:
- Perform thorough evaluations of patients' medical, social, and emotional requirements
- Deliver case management services to promote seamless care and effective resource utilization
- Track and assess patient progress, modifying care plans as necessary
- Collaborate with multidisciplinary teams to enhance patient outcomes
- Supervise patient electronic medical records to guarantee appropriate care delivery
- Generate reports aimed at minimizing patient care gaps and hospital readmissions

Qualifications:
- Valid LPN license required
- At least 2 years of experience in an acute care or managed care environment preferred
- Skilled in conducting patient evaluations and monitoring vital signs
- Strong communication and interpersonal abilities to effectively engage with healthcare teams
- Experience in Geriatric behavioral health is a plus
