Job Title: Licensed Practical Nurse

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Aveanna Healthcare LLC. As a Private Duty Nurse, you will provide high-quality care to our patients in the comfort of their own homes.

About the Role:
  • Implement physician-prescribed plans of care for patients
  • Conduct patient assessments and coordinate care with healthcare teams
  • Administer prescribed medications, treatments, and therapies
  • Document care provided in a timely and compliant manner
  • Teach and train family members on health promotion and disease management
Responsibilities:
  • Provide one-on-one care to patients with complex medical needs
  • Develop and implement individualized care plans to meet patient needs
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Active, unencumbered LPN license in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • 6 months prior hands-on nursing experience preferred
