Job Description: As an IC3 Care Coordinator with Aspire Behavioral Health & DD Services, you will play a vital role in supporting individuals with complex needs and their families. Your primary responsibility will be to coordinate and deliver person-centered care, working collaboratively with multidisciplinary teams to ensure seamless transitions and outcomes.

Duties and Responsibilities:
  1. 1. Conduct comprehensive intakes, assessing client needs and developing tailored care plans.
  1. 2. Coordinate with healthcare providers, social services agencies, and community resources to facilitate effective care delivery.
  1. 3. Provide emergency response services, offering crisis intervention and support when needed.
  1. 4. Maintain accurate records, documenting client progress, interventions, and outcomes.

Requirements: 2+ years of experience in social services, case management, or a related field, excellent communication and interpersonal skills, and ability to work effectively in a fast-paced environment.

Salary and Benefits: $50,000 - $65,000 per year, depending on experience, plus comprehensive benefits package, paid time off, and opportunities for professional growth and development.
