Job Summary

UnitedHealthcare is seeking a highly skilled and compassionate RN Case Manager to join our team in Finney County, Kansas. As a Field-Based RN Case Manager, you will play a critical role in ensuring the health and well-being of our Medicare and Medicaid members. Key Responsibilities
  • Assess, plan, and implement care strategies that are individualized and directed toward the most appropriate level of care for our members.
  • Utilize strong clinical skills to manage multiple tasks, prioritize responsibilities, and meet deadlines in a high-volume, high-priority environment.
  • Develop and maintain strong relationships with members, families, and healthcare providers to ensure seamless care coordination.
  • Proactively engage members to manage their own health and healthcare using motivational interviewing skills.
  • Manage transitions of care for members when discharged from in-patient hospital stays and emergency rooms.
  • Help members set goals and develop care plans to achieve those goals and update care plans regularly as they progress toward goals met.
  • Support members to improve their well-being by staying out of the hospital and attending regular visits to their primary physician and specialists.
  • Identify and initiate referrals for social service programs, including financial, community resources, medical, behavioral health, psychosocial, community, and state supportive services.
  • Communicate with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for members as needed to ensure they have access to care.
  • Provide clinical support for non-clinical staff of the team, including education on medications, diagnoses, and disease-specific education.
  • Collaborate with interdisciplinary teams to ensure appropriate services and identify needs.

Requirements
  • Current, unrestricted RN license in the state of Kansas.
  • 2+ years of experience working in the community as a RN in acute in-patient care, long-term care, home health, hospice, public health, behavioral health, or assisted living.
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook.
  • Ability to travel up to 75% of the time with reliable transportation, a current and non-restricted state of Kansas Driver's License, and State-required insurance.
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI).
  • Live in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service.
Preferred Qualifications
  • Bachelor's degree in nursing.
  • A background in managing Medicaid and Medicare populations with complex medical and behavioral needs.
  • Experience with arranging community resources.
  • Experience with electronic charting.
