Responsibilities:
Collaborate with a multidisciplinary team to provide comprehensive care to our patients
Develop and implement individualized treatment plans using evidence-based approaches

Assist with local community education and other duties as assigned
Maintain accurate and compliant documentation in accordance with Medicare guidelines
