We are seeking a skilled Occupational Therapist to join our team at Lone Tree Post Acute. As an Occupational Therapist, you will play a vital role in helping patients achieve their maximum potential and independence.

Key Responsibilities:
  • Develop and implement effective treatment plans to meet the unique needs of each patient.
  • Treat patients according to physician-approved treatment plans, ensuring the highest level of care and attention to detail.
  • Collaborate with the nursing department to train Restorative Aides and ensure seamless patient care.
  • Supervise Occupational Therapy Assistants in direct patient care and related activities, adhering to state practice act guidelines.
  • Communicate effectively with supervisors and healthcare team members regarding patient progress, challenges, and plans.
  • Participate in patient care conferences, utilization review meetings, and rehabilitation conferences as needed.
  • Conduct in-services training for other staff members to enhance their knowledge and skills.
  • Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation, adhering to Occupational Therapy Board state practice act and governmental requirements.
  • Recommend and facilitate the ordering of necessary durable medical equipment to promote patient independence.
  • Report any issues with department equipment to ensure it remains in good working order.
Requirements:
  • Valid Occupational Therapy license.
