**Job Summary:**

As an Occupational Therapist, you will be responsible for developing and implementing effective treatment plans for our patients.

**Responsibilities:**

* Develop and implement individualized treatment plans for patients
* Treat patients according to physician-approved treatment plans
* Assist the nursing department with training of Restorative Aides
* Supervise Occupational Therapy Assistants in direct patient care and patient-related activities
* Communicate with supervisors and other healthcare team members regarding patient progress, problems, and plans
* Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed
* Participate in in-services training programs for other staff in the facility
* Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, and treatment changes according to policy and procedures
* Instruct patient families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services
* Participate in discharge planning
* Recommend and facilitate the ordering of necessary durable medical equipment for patients to facilitate independence in mobility
* Report any problems with department equipment so that it is maintained in good working order

**Requirements:**

* Valid Occupational Therapy License
