Job Summary:
We are seeking a highly motivated Case Manager to serve single veterans in interim housing at a San Carlos hotel.
Under the supervision of the Program Director, the Case Manager will act as a strong support and advocate for clients, helping them attain stable housing and self-sufficiency.
The Case Manager will develop and implement individual case plans, assist with facility maintenance, and stay current on external community resources.
Responsibilities:
- Develop and implement individual case plans for a caseload of clients
- Assist the Program Director with facility maintenance and program services
- Stay current on external community resources, referrals, and service linkages
- Facilitate clinical or life skills groups
- Maintain client and program records
- Meet regularly with supervisor for supervision and exchange of pertinent information
Requirements:
- Bachelor's degree in social work, psychology, or related field
- Minimum of two years' case management experience
- Prefer experience working with homeless veteran population and/or individuals with serious mental health issues
- Proven ability to collaborate with community partners
- Understanding of issues faced by homeless individuals and marginalized populations
- Availability to work occasional evenings and weekends
