As a prominent not-for-profit hospice organization, Trellis Supportive Care has been committed to delivering outstanding end-of-life care for many years. This role provides the opportunity to engage in the comprehensive practice of hospice and palliative care, including home hospice services, inpatient care, community outreach, and educational initiatives related to hospice and palliative medicine.
Our mission at Trellis Supportive Care is to offer empathetic care for individuals facing life-limiting illnesses and their families, ensuring quality medical, emotional, spiritual, and social support wherever it is needed. Our team, consisting of over 350 specially trained professionals, collaborates to help patients live fully and comfortably, maintaining hope even during the most challenging times.
Key Responsibilities:
- Deliver direct medical care to patients, ensuring effective management of uncomfortable symptoms.
- Serve as an essential member of the interdisciplinary team, attending meetings as scheduled and acting as a medical resource for the team, guiding clinical service providers and ensuring adherence to medical standards of care.
- Certify that patients meet the medical criteria for hospice admission based on available diagnostic and prognostic indicators, current medical findings, and treatment orders.
- Re-certify patients as necessary for the continuation of hospice benefits at appropriate care levels.
- Ensure physician involvement in the development of each patient's hospice care plan.
- Participate in family meetings alongside the hospice team to educate patients and families about medical conditions and treatment options.
- Consult with the patient's primary care physician as needed regarding hospice eligibility and symptom management.
- Actively engage in the organization-wide Quality Assessment and Performance Improvement (QAPI) Plan.
- Stay informed about the latest research and trends in hospice care and pain/symptom management.
Qualifications:
- Must be licensed to practice medicine in North Carolina (MD or DO).
- A minimum of two (2) years of experience in medical practice or recent completion of a hospice and palliative medicine fellowship is required.
- Experience in hospice care is essential.
- Board certification in the specialty area is necessary.
- Board certification in hospice and palliative medicine is strongly preferred.
