We are seeking a dedicated and compassionate Physical Therapist to join our team of skilled home health care providers.

Job Summary

The ideal candidate will have a strong commitment to providing quality care to our patients in their homes. As a Physical Therapist, you will be responsible for providing Physical Therapy services to patients in compliance with all applicable practice acts.

Responsibilities
  • Develop and implement the interdisciplinary client care plan as it pertains to Physical Therapy.
  • Teach, supervise, and counsel the family and client in the physical therapy program.
  • Communicate with other team members and instruct them in certain phases of physical therapy.
  • Write clinical/progress notes on the date of the visit and incorporate same in the client's record according to company policy.
Requirements
  • Graduate of an accredited Physical Therapy program or equivalent education.
  • Licensed to practice as a PT in the State of Florida.
  • Home healthcare experience is preferred but not essential.
  • Electronic medical record experience is preferred but not essential.
