Job Title: Physical Therapist

Pinnacle Home Care Inc. is seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Job Summary:

The Physical Therapist will be responsible for assessing patients' physical abilities, developing and implementing individualized treatment plans, and providing skilled physical therapy services to patients in compliance with all applicable practice acts and agency policies.

Responsibilities:

  • Conduct comprehensive assessments of patients' physical needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement individualized treatment plans in consultation with physicians and revise as necessary.
  • Assist in the development and implementation of interdisciplinary client care plans as it pertains to physical therapy.
  • Teach, supervise, and counsel patients and their families in physical therapy programs.
  • Supervise Physical Therapist Assistants at least every 30 days.
  • Communicate with physicians and other healthcare professionals regarding patients' status and obtain additional orders as necessary.
  • Communicate with other team members and instruct them in certain phases of physical therapy as needed.
  • Document clinical and progress notes on the date of the visit and incorporate same in the patient's record according to company policy.

Qualifications:

  • Graduate of an accredited Physical Therapy program or equivalent education.
  • Licensed to practice as a PT in the State of Florida.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • At least one year of experience as a Physical Therapist.
  • Home healthcare experience is preferred but not essential.
  • Electronic medical record experience is preferred but not essential.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
  • Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
