Job Summary

We are seeking a compassionate and skilled Social Worker to join our team. As a Hospice Medical Social Worker, you will provide psychosocial evaluations, ongoing counsel, bereavement services, and community education outreach to patients and families in need.

Responsibilities
  • Conduct thorough psychosocial evaluations to identify patient needs and develop individualized plans of care
  • Provide ongoing counseling and support to patients and families to address emotional, spiritual, and practical needs
  • Develop and implement bereavement services to support patients and families through the grieving process
  • Collaborate with interdisciplinary teams to provide comprehensive care and support
  • Participate in community education and outreach efforts to promote hospice care and services
Requirements
  • MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
  • 2+ years of clinical work experience, preferably in healthcare or hospice
  • Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
