Individual Support Specialist

Position Overview:
As an Individual Support Specialist, you will play a vital role in delivering reliable and age-appropriate support to individuals and families. This position focuses on providing tailored assistance to participants and being an integral member of the Seven Hills Foundation team.
Key Responsibilities:
Engage in a pre-placement discussion with Family Services and the individual or family to create a personalized support plan.
Accurately complete time sheets detailing activities, secure family signatures, and submit these in line with the Seven Hills payroll timeline.
Deliver one-on-one support to individuals while adhering to designated hours.
Complete orientation and mandatory training sessions as stipulated by Seven Hills Foundation and the Department of Developmental Services.
Participate in job-related training as assigned.
Stay informed about the Seven Hills Foundation Support Worker Manual and organizational policies.
Support individuals in nurturing existing relationships and expanding their social networks.
Maintain regular communication with families, individuals, and Seven Hills Family Support personnel.
Consistently uphold an agreed-upon schedule.
Promptly report any changes or concerns affecting the individual or family to the designated Seven Hills contact.
Additional Responsibilities:
Encourage individuals to participate in community activities aligned with their interests as outlined in their Individual Support Plan (ISP) and provide opportunities to discover new interests.
Deliver a high standard of care to individuals, ensuring their safety, well-being, and dignity.
Qualifications:
A passion for supporting individuals with disabilities. Must be at least eighteen (18) years old. Familiarity with creative activities suitable for the age and abilities of the individuals being supported. First Aid and CPR certification is mandatory. A valid driver's license is required.
