Care Manager RN

We are seeking a skilled Care Manager RN to join our team at Kadlec Regional Medical Center. As a Care Manager RN, you will play a vital role in facilitating communication between caregivers and providers to advocate for our patients.
Key Responsibilities- Act as a liaison between staff and interdisciplinary teams to support coordination of patient care.
- Track the ordering and completion of studies and steps necessary prior to starting treatment to ensure timely authorization and treatment start.
- Coordinate and facilitate admission for in-hospital treatment and seamless transition back to outpatient treatment.
- Collaborate with providers to ensure timely start of treatment for best possible patient outcomes.
- Graduation from an accredited nursing program.
- Washington Registered Nurse License.
- 2 years Recent Outpatient Oncology Nursing experience.
- Bachelor's Degree Nursing, from an accredited educational institution.
- 2 years Acute Care Nursing experience.
