Care Manager RN

We are seeking a skilled RN Care Manager to join our team at Kadlec Regional Medical Center. As a RN Care Manager, you will play a critical role in facilitating communication between caregivers and providers to ensure timely and effective patient care.
Key Responsibilities- Act as a liaison between staff and interdisciplinary teams to support coordination of patient care
- Track and coordinate the ordering and completion of studies and steps necessary prior to starting treatment
- Coordinate and facilitate admission for in-hospital treatment and seamless transition back to outpatient treatment
- Collaborate with providers to ensure timely authorization and treatment start
- Graduation from an accredited nursing program
- Washington Registered Nurse License
- 2 years Recent Outpatient Oncology Nursing experience
