About the Role

We are seeking a skilled and compassionate Home Health Nurse LPN to join our team at Aveanna Healthcare. As a Home Health Nurse LPN, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Job Summary

The successful candidate will be responsible for delivering and coordinating quality patient care in compliance with physician orders. You will continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.

Key Responsibilities

  • Provide and document skilled nursing care under the supervision of a Registered Nurse
  • Observe and assess patient condition and care needs
  • Report changes in condition to the supervisor and/or physician
  • Document all activities, assessments, nursing actions, responses, and coordination of care
  • Participate in and update the nursing care plan

Requirements

To be considered for this role, you must have:

  • A graduate degree from an accredited school of nursing
  • A current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures

Prior Experience

While not essential, we welcome applicants with six months of recent experience as a Licensed Nurse in a clinical care setting. Home health experience is also desirable.
