Job Summary:

A dynamic opportunity is available for a skilled Licensed Practical Nurse (LPN) to join our interdisciplinary team at Providence Health & Services. As a Senior Home Health Nurse Practitioner, you will play a vital role in providing high-quality care to patients in their homes, working closely with an experienced team to create personalized care plans and deliver exceptional patient outcomes.

About the Role:

This full-time position offers a competitive salary of $75,000 per year, plus benefits and perks that support your well-being and career growth. Duties and Responsibilities:

- Provide comprehensive nursing care to patients in their homes, including assessments, medications management, and wound care
- Collaborate with the healthcare team to develop and implement individualized care plans
- Educate patients and their families on self-care techniques, disease management, and medication adherence
- Conduct home visits to assess patients' needs and provide ongoing support
- Document patient interactions and progress in electronic medical records

Requirements:

- Graduate of an accredited LPN/LVN school of nursing
- National Provider BLS certification within 30 days of hire
- Oregon Licensed Practical Nurse upon hire
- Valid driver's license and reliable transportation
- Ability to work independently and as part of a team

Preferred Qualifications:

- Experience in home health or hospice care
