Key Responsibilities
  • Evaluate patients and create comprehensive care plans tailored to their needs
  • Communicate effectively with patients, families, and the care team to ensure seamless care delivery
  • Collaborate with interdisciplinary teams to provide holistic patient care

Requirements
  • Graduate of an accredited school of nursing with a current state license as a registered nurse
  • At least 2 years of experience in clinical care settings, preferably in hospice care
  • Reliable transportation and ability to lift up to 100 lbs
