Nurse Practitioner

Job Summary:
We are seeking a highly skilled and compassionate Registered Nurse to join our team at WellSpan Health. As a Registered Nurse - VNA Home Health, you will provide professional nursing services to individuals in the comfort of their own homes, utilizing a process of nursing assessment, planning, care, and evaluation in accordance with physicians' orders, established plans of care, patient diagnoses, corporate policies and procedures, and the Nurse Practice Act.
Key Responsibilities:
- Carry primary care management functions for assigned patients, safely and independently administering prescribed treatments, medications, and nursing interventions based on assessment and plan of care.
- Independently recognize subtle changes in condition and modify plan of care accordingly, initiating emergency action in response to acute risk situations.
- Organize and prioritize time based on higher acuity patient assignments and increased workload, evaluating patient care and promoting goal-directed change to achieve patient outcomes.
- Evaluate and monitor the effectiveness of interventions and progress toward outcomes for more complex groups of patients, initiating interdisciplinary collaboration in evaluation of patient progress.
- Seek learning opportunities and resources beyond own work environment, incorporating new knowledge and skills acquired into practice, and promoting an environment conducive to professional practice, interdisciplinary collaboration, and patient and staff safety.
- Display conflict resolution and negotiation skills to maximize team performance, providing feedback in a positive and respectful way, and aware of and investigating events that led to conflict, recommending preventative measures or solutions.
Requirements:
- Diploma Program Required or Associates Degree in Nursing, RN Required
- 1 year Previous experience with the same patient population Required or Current VNA LPN transitioning to RN Required
- Licensed Registered Nurse Upon Hire Required or Registered Nurse Multi State License Upon Hire Required and Basic Life Support Upon Hire Required
