Momentum for Health is seeking a highly skilled and compassionate PD Rehabilitation Services Specialist I On-call to join our team. As a key member of our rehabilitation services team, you will provide mental health rehabilitation and case management services to clients in a field-based setting.
Key Responsibilities- Develop and implement individualized service and treatment plans for clients, working closely with clinicians and other professionals to ensure comprehensive care.
- Provide group facilitation and counseling services to clients, promoting community integration and socialization.
- Assist clients and their families in accessing community resources, including housing, employment, and education services.
- Coordinate and perform activities related to food preparation and kitchen management, ensuring a safe and healthy environment for clients.
- Complete clinical records and other documentation, ensuring compliance with agency, payor, and regulatory requirements.
- Master's degree in a mental health-related field and two years' full-time experience in the mental health field, or equivalent combination of education and experience.
- First Aid Certification or successful completion within 30 days of hire.
- Certification in writing 5150 holds or obtain certification within six months of hire.
- Purchasing and kitchen experience, including basic nutrition, menu development, cooking techniques, safe food storage, and kitchen sanitation practices.
- Demonstrated ability to work independently and in a small team environment, engage and communicate effectively with others, and manage multiple tasks and demands.
The PD Rehabilitation Services Specialist I On-call will work in a fast-paced, dynamic environment, providing services to clients in a field-based setting. The ideal candidate will be able to work effectively in a team environment, communicate effectively with clients and colleagues, and manage multiple tasks and demands.
