Job Description

We provide 24-hour care for individuals in need of sub-acute care, long-term care or hospice care. Our services include physical therapy, occupational therapy, speech and vocational rehabilitation.

Responsibilities:

  • ">
  • Assists RN in admission procedure and participates in the planning, implementing and evaluation nursing care.">
  • Sets priorities and delivers direct care to meet the needs of the assigned residents.">
  • Provides medications, treatments and performs nursing procedures safely.">
  • Documents per policy and procedure in resident's medical record.">
  • Participates in discharge regime and provides necessary teaching to resident and family on a regular basis.">
  • Demonstrates safety awareness and follows protocol for fire, disaster, infection control and other environment of care practices.">
  • Maintains competency and proficiency in nursing resident care standards.">
  • Attends all mandatory classes, and participates in the continuing education program and other appropriate learning experiences.">
  • Participates in performance improvement activities and other committees as assigned.">

Requirements

We require a currently licensed or otherwise duly authorized Licensed Practical Nurse in the State of New York.
