IntelyCare is seeking a skilled Registered Nurse (RN) to join our team of nursing professionals. As a Care Management Nurse, you will be responsible for providing high-quality nursing care in post-acute facilities, such as SNFs and ALFs.

Key Responsibilities:

  • Administer individualized treatments and medications to residents
  • Develop care plans in conjunction with residents and medical teams
  • Set up medical equipment and monitor their use
  • Go over care options with residents and family members
  • Meet and consult with physicians and other healthcare staff as needed
  • Draw blood, urine, and other fluid samples for lab work
  • Ensure nursing standards and facility policies are met
  • Accurately document interventions and medical condition histories
  • Help with investigations into incidents, ensuring they're reported accurately
  • Train nursing staff and supervise them

Requirements:

  • Completion of an approved nursing program
  • At least 4 months' experience in either post-acute care or in-patient hospital settings
