The successful candidate will be responsible for providing inpatient discharge planning, hospital experience, and working with Epic and Allscripts systems.

Key Responsibilities:

  • Develop and implement discharge plans for patients
  • Collaborate with healthcare teams to ensure seamless transitions
  • Manage a caseload of 10-30 patients independently
  • Provide Hospice, SAR referrals, LTACHs, Acute rehabs, and H.D. insurance authorizations
  • Work with abuse cases and shelter admissions

Requirements:

  • Masters in Social Work required
  • LMSW in NYS required
