Case Manager

We are seeking a highly motivated and compassionate Case Manager to join our team at Sound Community Services, Inc. (SCSI). As a Case Manager, you will play a vital role in supporting individuals with persistent mental illness and substance use disorders in achieving their recovery goals.
Key Responsibilities:- Assist individuals in accessing improved care coordination, linkage, referral, family support, and health and wellness promotion.
- Follow up on care and follow through on recommendations/referrals.
- Assist with appointment scheduling and accessing and transporting to services.
- Identify and address individuals' perceived barriers to treatment participation.
- Skill building/teaching/coaching to help individuals maximize independence in the community.
- Monitor individuals' participation in and response to treatment.
- Reach out to individuals on a regular basis to assess how they are doing.
- Associate degree or 60 credits equivalent in a related field, or certification in a medical or human services field.
- Minimum of two years of behavioral health or human services related experience.
- Current, valid Motor Vehicle Driver's license.
- Vehicle available for use during the workday.
- Competent user of technology, including email, agency intranet, and electronic client record.
- Demonstrated ability to work with clients in the field teaching skills necessary for community living and supporting recovery.
- Excellent written and oral communication skills.
- CPR/First Aid Certified or eligible.
- Bilingual (Spanish/English) preferred.
- Regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs.
- Regularly required to walk, talk, and/or hear.
- Speaking and hearing ability should be sufficient to communicate effectively by phone or in person at normal volumes.
- Vision adequate to read correspondence, computer screen, forms, etc.
