Job Title: Case Manager - Behavioral Health Homes Program

We are seeking a highly motivated and compassionate Case Manager to join our team at Sound Community Services, Inc. (SCSI). As a Case Manager, you will play a vital role in supporting individuals with persistent mental illness and substance use disorders in achieving their recovery goals.

Key Responsibilities:
  • Assist individuals in accessing improved care coordination, linkage, referral, family support, and health and wellness promotion.
  • Follow up on care and follow through on recommendations/referrals.
  • Assist with appointment scheduling and accessing and transporting to services.
  • Identify and address individuals' perceived barriers to treatment participation.
  • Skill building/teaching/coaching to help individuals maximize independence in the community.
  • Monitor individuals' participation in and response to treatment.
  • Reach out to individuals on a regular basis to assess how they are doing.
Requirements:
  • Associate degree or 60 credits equivalent in a related field, or certification in a medical or human services field.
  • Minimum of two years of behavioral health or human services related experience.
  • Current, valid Motor Vehicle Driver's license.
  • Vehicle available for use during the workday.
  • Competent user of technology, including email, agency intranet, and electronic client record.
  • Demonstrated ability to work with clients in the field teaching skills necessary for community living and supporting recovery.
  • Excellent written and oral communication skills.
  • CPR/First Aid Certified or eligible.
  • Bilingual (Spanish/English) preferred.
Physical Requirements:
  • Regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs.
  • Regularly required to walk, talk, and/or hear.
  • Speaking and hearing ability should be sufficient to communicate effectively by phone or in person at normal volumes.
  • Vision adequate to read correspondence, computer screen, forms, etc.
