Client Support Specialist

Role Summary
The Client Support Specialist plays a crucial role in providing outreach and assistance to individuals facing mental health challenges. This position requires a commitment to enhancing the well-being of clients through effective case management and support services.
Qualifications
A Bachelor's degree in social work, psychology, counseling, or a related discipline is required. Candidates should have at least one year of relevant experience following their degree. A valid driver's license is necessary for this role.
Key Responsibilities
- Deliver community-based outreach services to individuals diagnosed with mental health disorders.
- Conduct case management tasks, including connecting clients with mental health and other essential services, coordinating care, and aiding clients in formulating personalized service plans.
- Provide services in clients' homes or other suitable locations, and actively participate in team and community provider meetings.
- Maintain electronic clinical records as mandated by funding sources and accreditation standards.
- Achieve expected productivity levels for direct service delivery to clients.
Reporting Structure
This position reports to the Program Director or Program Coordinator within the Integrated Case Management Services.
