Position Overview:
We are seeking a Hospice Practical Nurse to deliver direct patient care and supportive services to individuals and their families. This role offers a flexible schedule and is designed for those seeking a part-time opportunity.
- Assess patient and family needs, contributing to the development and implementation of individualized care plans.
- Communicate any observations or changes in the patient's health status to the Registered Nurse, interdisciplinary team, and referring physician.
- Participate in team meetings to discuss patient care strategies.
Qualifications:
We are looking for dedicated and empathetic LPNs who hold a valid license to practice in the state and possess a minimum of one year of experience in patient care. A current driver's license, automobile insurance, and CPR certification are also required.
