Centralized Navigation Specialist

We are looking for a highly skilled and compassionate Centralized Navigation Specialist to join our team at Carolina Neurosurgery and Spine Associates. This full-time salaried position is based in mid-town Charlotte, North Carolina, and involves coordinating patient care and navigation for the Brain tumor program.
Key Responsibilities- Develop and implement individualized care plans to address patient therapy and treatment needs.
- Collaborate with multidisciplinary teams to prioritize timely communication and follow-up, ensuring effective care coordination.
- Obtain necessary authorizations and collect relevant information for appointments and admissions.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless transitions and high-quality patient care.
- Contribute to the development of evidence-based best practices and guidelines, promoting high-quality patient care.
- A valid NC RN License is required for this role.
- The ideal candidate will possess strong communication and teamwork skills, with the ability to adapt to changing assignments and responsibilities.
This is a full-time salaried position, Monday through Friday, with variable hours depending on patient needs.
