Job Title: Care Coordinator Specialist

About the Role:

We are seeking a highly skilled and compassionate Care Coordinator Specialist to join our team at Eventus WholeHealth. As a Care Coordinator Specialist, you will play a vital role in ensuring seamless transitions of care for our patients.

Key Responsibilities:

  • Client Caseload Management: Assist with delivering quality care for patients with chronic conditions by providing critical insights and valuable data to Eventus care teams.
  • Organizational and Administrative Duties: Facilitate discussions with healthcare providers to discuss client care plans, transitions of care, medication reconciliation, family input, and specialty referrals.
  • Patient Engagement: Call patients/POAs/caregivers within 48 hours after a discharge from a hospital or SNF to check in and schedule a follow-up visit.

Requirements:

  • Licensure and Certification: LPN degree or experienced certified MA with 5 years primary care experience.
  • Experience: 2 years minimum experience in the LTC/ALF setting.
  • Skills: Strong knowledge of geriatric population and patient navigation, excellent communication and interpersonal skills, organized with confidential client material, appointment tracking, and caseloads.

Estimated Salary Range: $60,000 - $80,000 per year
