At St. Croix Hospice, we are seeking a compassionate and skilled RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in providing high-quality end-of-life care to patients and their families.
About the Role:- Plan, organize, and direct hospice care with a focus on community health education and experience.
- Build relationships with patients and families to understand their unique needs and develop personalized care plans.
- Travel throughout the day and sometimes after-hours to provide care and support to patients in their homes and communities.
- Work collaboratively with an interdisciplinary team to ensure seamless care coordination and excellent patient outcomes.
