Job Title: Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our team at Elderwood of Scallop Shell at Wakefield. As a Physical Therapist, you will play a vital role in helping our residents achieve their rehabilitation goals and improve their overall quality of life.

Job Summary:

The Physical Therapist will be responsible for administering skilled physical therapy treatments to our residents, ensuring that they receive the highest level of care and attention. This includes developing and implementing individualized treatment plans, conducting assessments and evaluations, and providing education and training to residents and their families.

Responsibilities:
  • Administer skilled physical therapy treatments to residents, including exercises, modalities, and other interventions.
  • Develop and implement individualized treatment plans to meet the unique needs and goals of each resident.
  • Conduct assessments and evaluations to determine the resident's functional abilities and identify areas for improvement.
  • Provide education and training to residents and their families on proper techniques and exercises to promote independence and mobility.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
  • Document resident progress and outcomes in a timely and accurate manner.
Requirements:
  • Graduation from an approved school of physical therapy.
  • State license to practice physical therapy.
  • Two years of experience in physical therapy preferred.
  • Ability to direct others and work effectively in a team environment.
