About the Role

As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. You will work with patients to develop a treatment plan, perform interventions aimed at improving and enhancing their well-being, and evaluate their progress.
