Job Overview

Helios Behavioral Health is seeking a highly skilled Behavioral Health Clinician to join our team. As a core member of our Collaborative Care Model, you will play a critical role in providing evidence-based clinical support and interventions to patients enrolled in our program.

Key Responsibilities:

  • Provide brief, evidence-based clinical support and interventions to patients on an assigned caseload.
  • Screen and assess patients for common mental health and substance use disorders using validated screening tools.
  • Facilitate patient engagement and follow-up care.
  • Provide short-term, evidence-based therapeutic interventions such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and Solution-Focused Brief Therapy.
  • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
  • Work with patients to assess and address identified Social Needs such as food insecurity, transportation, and housing needs.
  • Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.
  • Coordinate with larger systems on patient's behalf, including when appropriate with schools, other medical providers, and/or family members.
  • Participate in regularly scheduled caseload consultation with a psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider.
  • Track patient follow up and clinical outcomes using a registry.
  • Document patient progress and treatment recommendations in Electronic Health Record (EHR) and other required systems to be shared with medical providers, psychiatric consultants, and other treating providers.
  • In consultation with the medical provider and the psychiatric consultant, facilitate treatment plan changes for patients who are not improving as expected and/or who may need more intensive or specialized mental health care.
  • Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are preparing to be discharged from the caseload.
  • Coordinate with existing clinic billing team members to bill for services.
