Case Manager Travel Nurse Job Description

We are seeking a skilled RN Case Manager to join our team of healthcare professionals at Travel Nurse Across America. As a Case Manager Travel Nurse, you will play a vital role in advocating for patients, evaluating their needs, and developing comprehensive care plans to ensure optimal outcomes.

Key Responsibilities:
  • Evaluate and assess new patients to identify their unique needs and develop personalized care plans.
  • Collaborate with healthcare teams, including doctors, social workers, and other RNs, to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
  • Analyze patient data to identify trends and areas for improvement, and develop strategies to reduce hospital stay lengths and readmission rates.
  • Properly document and submit medical records to ensure accurate and timely care coordination.
Requirements:
  • Current RN licensure and 2 years of recent case management experience.
  • Ability to stand and walk for long periods, carry up to 10 pounds, and bend, lean, and stoop without difficulty.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a collaborative environment.
