About the Role

We are seeking a skilled RN Case Manager to join our team at Providence Home and Community Care. As a RN Case Manager, you will be responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence.

Key Responsibilities
  • Provide evaluation and nursing services to patients in their place of residence
  • Function independently and/or in conjunction with an interdisciplinary team based on the patient's identified needs
  • Practice within the scope defined by the Washington State Nurse Discipline Practice Act, based on the nursing process model and concept of community health nursing therapy
Requirements
  • Graduate of an accredited school of nursing
  • Washington Registered Nurse License
  • National Provider BLS - American Heart Association
  • National Provider Identifier (NPI) number
Preferred Qualifications
  • Bachelor's Degree
  • 2 years of full time experience practicing as a registered nurse in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing Facility
  • 1 year of recent, hands-on technical clinical experience in a Medicare-certified Homecare or Hospice agency/facility doing intermittent visits and case management
