Nurse Clinician III, Outpatient Specialist

Key Responsibilities:
Provide direct patient care to patients in an outpatient setting
Develop and implement individualized care plans to meet the unique needs of each patient
Collaborate with the interdisciplinary team to coordinate patient care and achieve optimal outcomes
Educate patients and families on disease management and self-care techniques
Participate in quality improvement initiatives to enhance patient care and safety
Requirements:
Current RN licensure in the state of Texas
Minimum of two years of clinical RN experience
Current BLS certification
Ability to work independently and as part of a team
Strong communication and interpersonal skills
