Job Summary:

We are looking for a compassionate and experienced Registered Nurse to provide exceptional care to our patients in their homes. The ideal candidate will have a strong background in community health, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Conduct thorough assessments of patients' health needs and develop personalized care plans
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention
  • Collaborate with healthcare teams to coordinate care and ensure seamless transitions
  • Document patient progress and communicate effectively with physicians and other healthcare professionals
  • Participate in performance improvement initiatives and maintain a high level of clinical expertise
Requirements:
  • Graduate of an accredited nursing program
  • Active unrestricted license to practice nursing in the state of Texas
  • 2+ years of clinical experience in a community health or medical/surgical setting
  • Access to reliable transportation for travel to patient sites
  • Ability to work flexible hours, including evenings and weekends
