Job Summary:

We are seeking a skilled Registered Nurse to join our team as a Clinical Care Coordinator. In this role, you will play a critical part in providing direct and indirect patient care in a healthcare setting.

About the Role:

  • You will effectively communicate with physicians, nurse managers, charge nurses, and co-workers about changes in patient clinical conditions.
  • Ensure accurate and timely reporting of diagnostic studies and symptomatology.
  • Maintain a focus on patient safety and quality care throughout your interactions.
  • Foster effective communication among multidisciplinary teams to optimize patient outcomes.
  • Utilize analytical skills to interpret data and make informed decisions about patient care.

Responsibilities:

  • Assess patients' needs and develop individualized care plans that address their physical, emotional, and social well-being.
  • Collaborate with other healthcare professionals to coordinate patient care and ensure continuity.
  • Provide education to patients and families regarding their health status, treatment options, and self-care techniques.
  • Participate in performance improvement initiatives to enhance the quality and efficiency of patient care services.

Requirements:

  • A graduate degree from an accredited school of nursing, preferably with a BSN degree.
  • Current RN licensure in the state of New York and Basic Life Support certification.
  • Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities simultaneously.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare colleagues.
  • Proficiency in using electronic medical records and other healthcare technologies.

Salary:

$68,000 - $90,000 per year, depending on experience.
