LPN Homecare Nurse Specialist

MGA Homecare is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care in the comfort of their own homes. As a key member of our team, you will play a vital role in delivering specialized care, administering medication, and empowering patients and families to manage their health effectively.
Key Responsibilities:- Provide personalized care to patients, adhering to their active plan of care and ensuring optimal treatment outcomes.
- Administer prescribed medication, treatments, and therapies with precision and compassion, prioritizing patient comfort and safety.
- Document all activities, assessments, nursing actions, responses, and coordination of care meticulously, maintaining accurate and comprehensive patient records.
- Educate patients and families on disease management, self-care techniques, and prevention strategies, fostering understanding and empowerment in managing their health.
