Job Title: Licensed Practical NurseJob Summary

Eliot Community Human Services is seeking an experienced Licensed Practical Nurse to coordinate residential services for adults with developmental disabilities and brain injuries. The successful candidate will have 3 years of related experience and skills to establish supportive relationships with individuals and provide quality nursing care.

Responsibilities
  • Medical Consultation and Education: Provide education and consultation to individuals and staff on medical issues, treatments, medications, wellness, and prevention.
  • Program Oversight: Provide oversight, guidance, and consultation in all residential programs around medical protocols, medication management, and coordination of care.
  • Medical Documentation: Review all medical documentation, complete medical assessments, and prepare medical documentation for all new referrals and discharges.
  • Nursing Care: Perform direct nursing care and medical support services to all residential programs as needed.
  • Staff Supervision: Hire, train, and supervise nursing staff for the Brain Injury programs.
  • Community Partnerships: Develop working relationships with community medical providers and VNA-related services to assist with coordinating medical care for individuals.
  • Treatment Planning: Participate in the development of treatment goals for each individual related to medical, medication, or health concerns.

  • Commitment to meeting the individual needs and preferences of each person served
Benefits
  • Full-time position, Monday to Friday, 8am-4pm or 9am-5pm
  • $62,680.00 salary
