Job Summary:

As a Nurse Practitioner with Fallon Health, you will play a vital role in providing high-quality, patient-centered care to our elderly adult population. We are seeking a skilled and compassionate healthcare professional to join our team and make a meaningful difference in the lives of our members.

About the Role:

This is a full-time position that will involve managing a primary care panel of participants and providing continuity of care to PACE participants residing in the community or long-term care setting. You will work closely with the interdisciplinary team to establish realistic goals and develop individualized plans of care that focus on disease prevention, wellness, and management of chronic and episodic illness.

Responsibilities:

* Perform post-enrollment visits and biannual exams of participants, identifying and documenting problems/issues in medical records
* Evaluate episodic illnesses and order diagnostic and therapeutic interventions as needed
* Collaborate with physicians and other team members to manage episodic and chronic medical needs of participants
* Address and document Advance Directives/MOLST discussions for participants annually and when a change in condition occurs
* Perform diagnostic, therapeutic, and nursing interventions as indicated
* Provide continuity in monitoring of chronic, stable illnesses
* Implement participant plans of care in a safe and effective manner
* Participate in interdisciplinary team meetings and evaluate the effectiveness of previously implemented treatments
* Conduct home and nursing home visits as needed
* Maintain accurate documentation and coding of services provided according to professional standards
* Identify utilization and service delivery problems and participate in resolving them
* Participate in quality assessment activities, including record reviews, outcome measurement, and review/revision of policies and procedures

Requirements:

* Master's degree in Nursing required, with a graduate degree from an accredited Nurse Practitioner program
* Licensed by the Commonwealth of Massachusetts Board of Registration in Nursing
* One to three years of Nurse Practitioner experience preferred, with at least one year of experience in geriatric care
* Experience working in a healthcare setting as a member of a professional clinical team
* Experience with care coordination
