Job Summary:

The Hospice Social Worker - Patient Care Specialist will provide comprehensive social work services to patients and their families in accordance with the individualized plan of care. This includes coordination of care, counseling, and education to ensure the best possible quality of life for our patients.

Responsibilities:

  • Participate in ongoing interdisciplinary comprehensive assessments to develop and evaluate the Plan of Care.
  • Contribute to patient and family counseling and education to address their unique needs and concerns.
  • Collaborate with the hospice care team to ensure seamless coordination of care and services.
  • Participate in the QUAPI program and hospice-sponsored in-service training to stay up-to-date on best practices and industry developments.

Qualifications:

  • Master's degree in Social Work from a school accredited by the Council on Social Work Education.
  • License or license eligible preferred, but new graduates with an MSW will be considered.
  • Valid driver's license and access to a vehicle during working hours.
